Once again a tour guide is a must for first timers. Absolutely breath taking.
The Basilica and St. Peter's Square are well worth the time. Go with a Skip the Line Tour. Book ahead.
Don’t buy tickets from outsiders. You can walk inside and buy tickets at low price.

They have services on Wednesdays so we could not tour there. Saw the outside only and it was crowded with people.
If you decide to climb the steps up into the dome, do so early in the day while you are fresh. The views are worth it but the hike is a significant one. Also consider entering the basilica when you see that the line to get through security is noticably shorter than it had been. Hotel staff can often advise when the pontiff will be delivering a blessing.
